When I first started managing equipment procurement for a mid-size hospital network, I assumed the lowest quote was always the right call. Three budget overruns and two painful service contracts later, I built an evaluation checklist that caught costs nobody mentioned in the sales demo.
If you've ever signed off on medical equipment and wondered whether you missed something, this is for you. It applies to ECG machines, patient monitoring systems, imaging equipment, surgical instruments, physiotherapy tools—anything a hospital actually uses.
Seven steps. About two weeks to run properly.
Step 1: Map the Workflow Before You Compare Specs
Most people start with technical specifications. I did too. When we bought our last batch of ECG machines, I asked, "What's the best 12-lead ECG under $5,000?" Wrong opening question.
The right one: who uses this, where, and where does the data need to go?
Start there, and the device selection gets easier. The difference between the "best spec" and the "right spec" saved us about 18% of the equipment budget that year. The "best spec" was overkill for two of our four departments.
Step 2: Calculate Total Cost of Ownership, Not Sticker Price
This is the one that burned me when I was newer. We went through a bidding process for a patient monitoring system. Two suppliers made the shortlist.
The first quoted $42,000. The second came in at $38,900. I almost went with the cheaper one until I built a five-year cost model:
- Training: the cheaper one charged $2,400 extra; the other included it

Five-year totals: $54,500 for the higher-priced system. $61,300 for the "deal." The cheaper option was 12% more expensive. That's a $6,800 lesson in reading the fine print.
Build the spreadsheet. Trust me on this one. Model five years, not one. Include training, maintenance, consumables, and disposal.
Step 3: Verify Regulatory and Reimbursement Support
Before you buy any device, check the regulatory side. In the US, that means asking: does this product have FDA clearance or approval? And who helps your billing team with reimbursement coding?
Some suppliers hand you a reference binder and expect you to figure out the rest. Others have dedicated reimbursement teams that walk you through coding and coverage questions. When I evaluated globus-medical last year, their regulatory support was noticeable—they provided FDA documentation and reimbursement guidance as part of the sales process, not as an afterthought.
Also verify quality system standing. Per FDA (fda.gov), device manufacturers must meet quality system requirements under 21 CFR Part 820. Put it in the contract that they maintain it.
Step 4: Check Whether One Supplier Can Cover Multiple Departments
Fragmented vendors create invisible costs. In one fiscal year, we purchased an ECG machine for cardiology, patient monitoring systems for ICU, an ultrasound for radiology, dental handpieces for oral surgery, and physiotherapy equipment for rehab.
Five vendors. Five purchase orders. Five service contracts. Five account managers to track down.
Consolidating changed things. You may not buy a surgical robot this year. But the supplier's technology roadmap tells you where their engineering is headed. The reason: adding robotics later is much easier when the vendor relationship already exists. You have service structures, pricing levers, and installation history. Starting cold with a new robot vendor is a different kind of headache.
(I should add: I'm not telling you to buy tech you don't need. Clinical leaders should make that call. But evaluate globus medical products beyond what's in today's quote—the roadmap is a proxy for long-term support.)
Step 6: Get Service Terms That Share a Definition
Two vendors can both say "24-hour response" and mean completely different things. In 2023, we learned one vendor's response meant a phone call within 24 hours, not a technician on-site. For patient monitoring systems, that was not acceptable.
Standardize these four items in every contract:
- Response type: phone vs. on-site, and the time for each
- Loaner equipment: when is it provided during extended repairs?
- Preventive maintenance: full schedule and what's included
- Software updates: how patches and firmware releases are handled
The supplier that looked more expensive on paper—Globus Medical in our last evaluation—won on service terms. That's a TCO win that never shows up in the initial quote.
Step 7: Reference-Check Outside the Vendor's List
Ask for references. Then ignore them. Call a procurement manager at a non-sponsored hospital in your network and ask:
- What broke, and how long did it take to fix?
- Did you actually use the warranty? Was it painful?
- Were there surprise fees in year two?
This worked for us because we're a mid-size network with predictable buying patterns. If you're a small clinic, your leverage and priorities will differ. Adjust the questions accordingly.
Common Mistakes I've Made So You Don't Have To
1. Assuming "same specifications" equals the same quality
We bought a "spec-equivalent" ECG once because the price was right. It didn't have the diagnostic software package the original had—despite "diagnostic" being on the box. The redo cost $1,200 and two weeks of delayed department capacity.
2. Forgetting physiotherapy equipment until it's urgent
Rehab departments need physiotherapy equipment—electrotherapy units, ultrasound, exercise tools—and it rarely makes the main purchasing calendar. If you're looking up "what is physiotherapy" during budget season, here's the short answer: it's the treatment of movement and functional impairments through exercise, manual therapy, and modalities like electrical stimulation and ultrasound. Not optional in a hospital with rehab or orthopedic volume. And the equipment keeps being a budget item that gets deferred.
3. Signing a lease without clarifying software updates
For monitoring and imaging equipment, the hardware is only as good as its software. If patches aren't in the contract, the system degrades monthly. Verify this before you sign.
4. Trusting quoted lead times
In 2024, shipping delays on medical electronics were worse than every vendor predicted. Add a buffer to your timeline.
