Step 1: Verify who you're buying from before you get attached to a product

A website can be put up in an afternoon. I'm not joking. When a hospital calls me in a panic, the first thing I check isn't the product spec. It's the registration. In the US, any medical device manufacturer, importer, or distributor must register with the FDA's Establishment Registration and Device Listing system (21 CFR Part 807).

Ask for the FDA registration number. Then verify it yourself in the FDA's medical device registration listing database. If they can't give you a number, or the number doesn't match the company name, stop. It doesn't matter how good the price is.

Why does this matter? Because in an emergency, the supply chain gets crowded with brokers. Some are fine. Some resell used equipment as “new surplus.” The FDA database is free, and it answers a question a brochure can't.

Step 2: Define the actual product spec

This is where the conversation usually stalls. A sales rep hears “we need a ventilator” and starts quoting. But what kind of ventilator?

  • Mechanical ventilator: a machine that moves air in and out of the lungs when a patient cannot do it themselves. Invasive or non-invasive? ICU or transport? The mode selection matters. The WHO's technical specifications for invasive ventilators list mandatory minimum modes and alarms, and it's a good baseline even today.
  • Fetal monitor: a device that tracks the baby's heart rate and uterine contractions. Some are handheld Dopplers; some are networked systems for a labor ward. The probes are usually proprietary, so compatibility with your existing monitors is the first question.
  • PCR machine: a thermal cycler that amplifies DNA segments. The question isn't just “how many wells” but what assays you'll run and whether the machine is cleared for those specific diagnostic uses. In the US, it also has to fit under your CLIA certificate.

Don't apologize for asking these questions. Every hospital I've supplied has at least one device in storage because someone bought a model that didn't match the existing system. Defining the spec before asking for price isn't bureaucracy. It's the fastest way to avoid an expensive mistake.

Step 3: Get the lead time in writing

“It's in stock” covers a wide range of realities. Sometimes it means the product is in a warehouse, ready to ship today. Sometimes it means it's on order from the factory and the rep is fairly confident it'll arrive in a few weeks. Ask specifically: “Can you send me the delivery date in writing before I issue the PO?”

In March 2024, a client called 36 hours before a funding deadline. They needed a fetal monitor for a research project—normal lead time for that model was 45 days. The vendor they'd been talking to said “should be fine.” We found a different supplier with a unit in the US, paid $700 extra for overnight freight, and the grant was saved. The difference between a verbal estimate and a written date.

Step 4: Compare total cost, not list price

The cheapest quote is often the most expensive purchase. That's not a slogan. It's a pattern I see whenever a buyer compares only the unit price.

Take a fetal monitor. A budget model might be $2,800 cheaper than a mid-range option. But are the probes included? What about replacement sensors, which you'll need every year or two? What about training night-shift staff on a system that behaves differently from your existing one? Add those up and the “savings” disappear.

A hospital client learned the hard way. The numbers said go with a cheaper PCR machine—15% lower price, similar well count. The specs technically matched. But something felt off: the manufacturer had no service engineer in the country. When the machine failed six weeks later, the repair required an engineer to fly in from overseas. The total cost ended up higher than our quote, and the lab was down for a week.

Price the total cost over three years. That's the only number that matters.

Step 5: Ask what happens if it breaks

It's not a question of if equipment breaks. It's when, and how fast it gets fixed. For a mechanical ventilator, downtime is a life-safety issue. Ask for:

  • Service response time in hours, not days
  • Where the service depot is located
  • Whether a loaner unit is provided during repair
  • Warranty terms for consumables and replacement parts

We didn't have a formal approval process for this at the start. It cost us when a client paid a rush fee to get a monitor “fixed” that just needed a $2,000 replacement cable. The cable was in the box. Nobody thought to check. A simple service-support checklist fixed that.

Step 6: Ask for clinical references you can actually call

Brochures are not references. The question “can I call one hospital that has used this exact model for at least a year?” is the best filter I know. A legit supplier will give you a reference. A reseller won't, because the installation record is scattered.

When you get the reference, call them. Five minutes: “Has the equipment been reliable? Is the service rep responsive? Would you buy it again?” That last question tells you more than any spec sheet.

Step 7: Put everything in the purchase order

Not the prettiest step, but the one that saves your budget. Every promise — delivery date, accessories, setup, training, service terms — must appear in the purchase order. If a sales rep says “that's not standard,” ask them to add it as a line item.

In my experience, buyers who skip this step pay rush fees twice: once for the equipment, once for the part the supplier “forgot” to include.

Three mistakes that show up after the checklist

  • Buying from a broker instead of the authorized distributor. The warranty only applies if the seller is allowed to sell it. Check the manufacturer's website for authorized distributors, or ask the manufacturer directly.
  • Ignoring the consumables pipeline. A PCR machine that uses proprietary tubes or reagents becomes a recurring cost you can't avoid. Ask what consumables are required and how much they cost before you sign.
  • No training plan. A mechanical ventilator is only useful if the night shift knows how to set it up. A fetal monitor is only useful if the nurse can connect the probes. Confirm training is included and covers the shift change.

Seven steps. It takes about an hour to run through them. That hour is a small price to pay for avoiding a mistake that shows up in a review meeting later. If a supplier is too busy to provide registration numbers, a written lead time, or a three-year cost breakdown, they're telling you how they'll behave.

Slow down for one hour. It's the fastest decision you'll make.
